Test Description

ISO Pyrogen – Material Mediated testing evaluates whether a medical device or its materials can induce a fever-causing (pyrogenic) response in the body due to non-endotoxin substances. Unlike bacterial endotoxin tests, this method detects pyrogens from sources like chemicals, fungi, or Gram-positive bacteria. It is typically performed in vivo using rabbits to monitor body temperature changes after exposure to device extracts. This test is essential for ensuring the safety of implantable or blood-contacting devices, where pyrogenic reactions could pose serious health risks.

Why This Testing is Performed

This test is conducted to:

  • Detect non-endotoxin pyrogens that may not be identified by traditional bacterial endotoxin tests.
  • Prevent febrile reactions in patients caused by material-mediated contaminants.
  • Fulfill regulatory requirements for biocompatibility, especially for devices that contact blood or cerebrospinal fluid.

Steps in Conducting This Test

  1. Extract Preparation: Device materials are extracted in a suitable solvent under controlled conditions.
  2. Animal Exposure: The extract is injected into rabbits via the intravenous route.
  3. Monitoring: Rabbits’ body temperatures are monitored over a defined period (3 hours).
  4. Evaluation: A rise in temperature beyond a specified threshold indicates a pyrogenic response.
  5. Interpretation: Results are compared against acceptance criteria to determine material safety.
